This page contains all the faculty forms ENA will need to obtain Contact Hour approval and order the necessary equipment for your presentation(s). Please complete one of each of these forms for each presentation. We have also included informational forms for your guidance.

Important Faculty Deadlines

2010 Annual Conference PowerPoint Presentation Submission

Faculty are required to submit presentations to AVMG by September 15, 2010.

    The Importance of Pre-submission

    Pre-submission allows AVMG to review the presentations ahead of time and contact speakers who have issues such as MAC to Windows conversions, missing video clips, or images, video or images in the wrong format, timing and animation issues, external links, etc. If faculty do not pre-submit and simply brings their presentation with them AVMG may not have time to correct some of the problems that may have been corrected in advance of the meeting.

    Presentation Updates and Changes:

    Online – Changes can be made to your presentation by re-submitting your file(s) before the September 15, 2010 deadline.

    Content and Format without Commercial Bias

    Educational materials that are part of a CE activity, such as slides, abstracts, handouts and monographs, cannot contain any advertising or product-group messages, or use trade names exclusively or inappropriately.

    All educational offerings will be free of commercial bias either for or against any products.  Any products discussed in an educational offering will be objective, balanced, and have scientific merit.  If trade names are used, those of several companies should be used rather than only that of a single supporting company.  When an unlabeled use of a commercial product, or an investigational use not yet approved for any purpose is discussed during an educational activity, the speaker must disclose that the product is not labeled for the use under discussion or that the product is still investigational.

    All presenters and moderators will be required to complete an ENA Conflict of Interest Statement.  ENA will disclose to program participants prior to the presentation those presenters or moderators that have a specific relationship with the sponsoring company.

    An important part of your presentation is your handout. Participants use this information for current and future reference material and depend on you to be accurate and up-to-date. In order to provide our participants with a well-rounded educational experience, we ask all faculty to include, as part of your course, a separate handout that will be made available to attendees in advance of the conference on a special Annual Conference section of the ENA Web site. Content outlines submitted to ENA will be substituted in place of handouts, if handout information is not received at ENA National office by the deadline of Monday, May 10, 2010.  

    ENA will not accept Powerpoint presentations or slide handouts as handout information.
