Practical Guide to Safer Handoff of Older Adult Patients between Long-Term Care Facilities and Emergency Departments
These valuable guidelines provide emergency nurses, long-term care nurses, and emergency medical services professionals with tools to ease the development of standardized communication processes to improve older adult patients’ handoffs.
